MAMA’S KITCHEN CKD PROGRAM

@Mamaskitchen is a wonderful nonprofit organization that for 30 years has provided home-delivered, medically tailored meals and nutrition services at no-cost to San Diegans battling critical illnesses. Their Chronic Kidney Disease Pilot Nutrition Program provides free home deliveries of three nutritional meals a day for every day of the week!

Individuals must be referred by a medical case manager, medical social worker or provider. Additionally, the referral form must be signed by a provider. Learn more about eligibility and referrals by visiting MamasKitchen.org
