Ready to talk with a mentor? You can submit a referral yourself, or ask your dialysis unit social worker to send one for you.
Next, a ReMend Intake Specialist will contact you to hear a little about your kidney journey and what support would be most helpful. We’ll match you with a mentor, and they’ll reach out directly. You’ll decide together how to connect—phone, video (Zoom/Microsoft Teams), or in person.
